That is a handoff failure, and it is the most common systems problem in a small kitchen precisely because a small kitchen has fewer people to notice one.
3. The Mechanism
What a brigade actually solves
The brigade is a solution to a coordination problem, not a hierarchy for its own sake.
It assigns production, so every item has an owner.
It assigns authority, so under pressure there is one person whose call settles a question.
It assigns information flow, so someone knows the state of the whole and can sequence against it.
Those three functions survive the collapse into two people. What disappears is the explicit assignment, because in a small kitchen it feels obvious — and obvious is exactly what fails at nine o'clock.
The handoff
A handoff is the transition of a product or a piece of information from one owner to another.
Production is usually assigned. Handoffs usually are not.
Which produces a specific failure shape: a product sits, finished, at a boundary, and nobody is late. The cook who made it considers it done. The person who should take it has not seen it. Neither one is failing at their job, and the plate is three minutes older than it should be.
A handoff with no owner is a handoff that runs on habit, and habit leaves with a person, a schedule change, or a busy night.

A standard transmitted verbally loses a detail at each retelling, and nobody is comparing against a reference.

One station runs beautifully and only when one person is on it.

Specific to this room and worth naming as its own case.

When does slicing start. Who checks the hold. What happens if the room fills early. Those are handoffs, and they are typically owned by habit.

Influenced indirectly: how quickly a problem surfaces, through whether the system asks or waits.
Observed and responded to: where product accumulates; who is quiet.
