Follow these steps
1 Start with the header
Record the venue or event, date, approximate times, exact location, preparer, and role.
2 Write only observable facts
Describe what was seen or heard. Avoid diagnoses, motives, blame, and unsupported conclusions.
3 Record actions and notifications
State what staff did, who was contacted, and when each action occurred.
4 Preserve evidence and follow-up
Record witnesses, available statements, harm, evidence location, owner, due date, and next action.
5 Review the live record
Check the preview for missing sequence, vague wording, or details that belong in an official system rather than this browser draft.
6 Copy, print, and transfer
Copy the summary or save the print view as a PDF, then move the record into every required official reporting system.

How saving, reopening, exporting, and printing work
Reopen this tool on the same browser and device through Saved Work. Browser drafts are not automatically synchronized to another phone or computer.
Opens the polished print workflow. In the browser print window, choose Save as PDF or select a printer.
Copies the completed summary for an approved record, message, or document. Review the copied text before using it.
Removes or resets working entries. Save, export, copy, or print the current work before using this control.
Tool-specific note: Fields save automatically in this browser. “Copy summary” puts a plain-text version on the clipboard. “Print / Save PDF” opens the browser print workflow. “Start a new report” clears the locally saved draft, so save or transfer the completed record first.
What the browser draft keeps: the entries in the tool fields. A generated calculation, preview, copied summary, exported file, or PDF is a separate result and may need to be reviewed or regenerated after reopening.
After reopening: review the restored fields, confirm dates and assumptions are still current, and regenerate or recheck the finished result before using it.
