The decisions
What to sign off on. An observed outcome, not attendance and not the trainee's own assessment.
Because "do you understand?" has verified nothing, and a sign-off on an event is a record of an event rather than of a capability, per Module 53.
When to stop supervising. After the unsupervised run has been compared to the reference — not before.
What you are reading
The output, unsupervised.
And whether they can say what they were reading — which is the test that separates trained from exposed.
What goes wrong here
Signed off on attendance.
And supervision withdrawn before verification.
